About the Role 

We are looking for an experienced and dynamic Lifestyle Assistant with a focus on achieving the best outcomes for our residents to join our home at Bayswater North. We are looking for people who genuinely care, show initiative and are able to present innovative ideas to the team. Our lifestyle program is one that we are proud of and ensures the wants and needs of our residents are met.  

 
Reporting directly to the Lifestyle Coordinator and Director of Nursing you will:

Implement delivery of activities to residents in a group setting and 1:1
Carrying out lifestyle assessments of residents  
Complete accurate documentation 
Liaise with residents and their families.
Assist with other tasks as directed by the Lifestyle Coordinator

About You 

Certificate IV in Leisure and Health
Excellent observation, attention to detail and reporting skills? 
Clear spoken and written skills??
Previous experience in a similar role
Demonstrated understanding and knowledge regarding the ACFI Framework (desirable)  
Experience using a computerised Care Management System (AXIS knowledge desirable)    
Outstanding computer literacy.  
Current NDIS Workers Screening check
Current COVID-19 Vaccinations
Current Flu Vaccination
